If you are looking for the ports for a different application you can find it by either:
Enter a number that has not yet been used into the ID box.
If you are only forwarding a single port number then enter that number into both the Private Port, Public Port Start and the Public Port End boxes. Choose the protocol that you are forwarding from the Port Type radio button.
In this router if you need both TCP and UDP forwarded then you need to create a duplicate configuration. One of the configurations has TCP selected and the other one has UDP selected.
You need to enter the IP address that you are forwarding ports to in the Host IP Address box. It's possible for this to be either the IP address of your computer or any other IP address on your network that you want ports forwarded to.
